‘Trailblazing’ East Lancashire plan to support mental health patients

It’s hoped this new approach will free up the hospital beds sooner and assessing the person in a more natural environment to help identify how independent they are

I’m Matt Rosser with news that a new scheme in East Lancashire to support mental health patients on their discharge from hospital has been described as ‘trailblazing’. Patients who are ready for discharge will be supported in a care home for six weeks by social workers, mental health nurses, a residential care team, occupational therapists and physiotherapists before they return home, or to a longer-term care setting.
